Assignment Form for the Preparation of an Asbestos Inventory for an Asbestos Certificate
What is this form for?
By means of this form, the client commissions a certified asbestos expert to prepare an asbestos inventory for the purpose of obtaining an asbestos inventory certificate (commonly referred to as the asbestos certificate. The purpose of the asbestos certificate is to make a formal statement regarding the asbestos safety of the inspected area. ).
This form formalises the assignment agreement as described in the inspection protocol. By completing this form, you fully describe your assignment for carrying out a standard investigation in accordance with this inspection protocol. The conclusions of the asbestos certificate may have significant consequences for the owner. Therefore, ensure that the asbestos expert has all the necessary information and documents to correctly prepare the asbestos inventory. For this reason, please complete the form as accurately and completely as possible to avoid any problems at a later stage.
This form only regulates the obligations of the client and the asbestos expert insofar as they implement the provisions of the Inspection Protocol. It is up to the parties themselves to supplement this form with an additional agreement in which they regulate all other mutual obligations, for example regarding prices and delivery deadlines.
The client shall provide all relevant information and documents to the asbestos expert. Please do so using the list in Title 9 of this form. The asbestos expert will assess the usability of the data supplied.

What is this form for?
